| All non-immigrant international students enrolled at the university are required to be insured. The insurance premium is included in the tuition fees. Insurance is covered semester by semester. Students not enrolled for a semester are not covered in any insurance plan. Should a student decide to buy just the insurance for the semester he or she is not enrolled in, he/she can purchase the accident and sickness insurance by contacting the Office of International Affairs. Monumental Life Insurance provides the insurance service. UL Lafayette students can access the company online and check the coverage status and obtain insurance ID card. In order to access the online service visit the company, and need to have the first name, last name and the social security number. Students may also call Bollinger, Inc. at (800) 526-1379; or visit the company online at http://www.bollingerinsurance.com/ullafayette. The information brochure contains most of the information regarding insurance policy. If you want to claim a medical expense, you need to fill out the Claim Form. It is a good idea to check whether your claim is valid from the information brochure. |

| To purchase additional insurance to get more coverage, please contact the company. If a student wants to buy an insurance plan for J-1 visitor exchange students, the dependents such as a spouse and/or children under 21 years of age can complete the additional insurance application, and mail it to Bollinger, Inc. |
